This executive summary reflects the continuing evolution of my work in multidisciplinary rehabilitation and the management of complex chronic illness and disability.
Building upon earlier clinical research and experience, it explores a coordinated model of care incorporating multidisciplinary healthcare, structured case management, patient education, nutritional and lifestyle interventions, ongoing monitoring, and continuing support.
This work further broadened my understanding of the interconnected nature of chronic disease and contributed to the continuing development of the clinical principles that would ultimately inform The Fowler Protocol™.
